Surgical Care Plan

The talking points below can be used when speaking with patients about the SeamlessMD program to explain its purpose, use and benefits.

“SeamlessMD is an interactive step-by-step guide to help you prepare for your surgery and recover faster afterward. You and/or a caregiver can access the online program on any device you’re comfortable with - a smartphone, tablet, laptop, or desktop computer are all great options. On your chosen device, you can use a web browser like Chrome, Safari,  or Microsoft Edge or you can download the SeamlessMD app. You can even move between different devices and your information would still be in sync.

Patients who have used SeamlessMD report feeling more confident and less worried at each stage of their healthcare journey. Research shows that patients who use the online program are less likely to visit an emergency room or need unexpected hospital stays as SeamlessMD helps you manage your health concerns early - before they get serious.

The program will guide you through all 3 stages of your surgery – getting ready for surgery, in-hospital and recovering at home after surgery.

You will receive real-time reminders about self-care tasks you need to complete, for example, when to stop eating or drinking, getting out of bed, and walking. These reminders will help you to stay on track and heal faster. You will have access to a library of education with different topics regarding your journey before, in-hospital, and after surgery. At home after surgery, you will have access to a daily Health Check survey. The daily Health Check will tell you if you are on track with your recovery, give you some self-care tips to manage your symptoms, or it might tell you to call your healthcare team for more support. Your healthcare team designed the content, so it’s important to follow those instructions to stay safe.

[OPTIONAL: Your healthcare team is monitoring the program and the program might alert them if you have problems. Don't wait for someone from your team to call you back as they might not see it right away]"

On your chosen device, you can use a web browser like Chrome, Safari,  or Microsoft Edge or you can download the SeamlessMD app. You can even move between different devices and your information would still be in sync.

During your time on the program, Seamless will:

  • Give you real-time reminders about tasks you need to complete
  • Provide you with a library of education to teach you how to take care of yourself and to help address concerns you may have
  • Ask you to complete a Health Check, which will let you know how you are doing and what action to take if needed

Seamless will let you know what to do based on your answers to the surveys. SeamlessMD Common Patient Responses

R: Where does the information within SeamlessMD come from?

A: All content and advice on the program was created and approved by the clinical team at HOSPITAL, including review by physicians.

R: Why does my email address or phone number need to be added to the program?

A: We must use either your email address or cell phone number to ensure that you receive your initial setup link and can access your account. Additionally, you will receive reminders from the program via email or text message depending on the one you’d prefer to have on file. After setting up your account, you can turn off your notifications if you don't want to receive future messages from SeamlessMD. You can still access your account on your own time with your email/phone number and password.

R: I’ve already used SeamlessMD for a previous care plan and don’t want to use it again.

A: We recommend that you use SeamlessMD again for your upcoming care journey as your preparation and recovery phases can look different even if you are enrolling on the same type of care plan. Since your recovery may differ from your previous experience, you will be able to report on new symptoms or concerns that you have and make use of the self-care tips.

R: I am not comfortable using technology, do not have a cell phone or email address, or do not have a device that would allow me to use the program.

A: SeamlessMD offers the ability for a family member or friend to be added as a caregiver on your program. These individuals can assist you in using the program and will receive reminders on your behalf to their own email address or phone number. Additionally, SeamlessMD has a support team that is available in the event you experience any technical issues with the program.  Most patients who initially report not being tech-savvy can successfully use SeamlessMD. Our average user is between the ages of 50-80 years of age. The technology is designed to support people of all levels of tech comfort.

R: I don’t think I will be able to use the program due to a disability or inability to use devices with a screen.

A: SeamlessMD meets and exceeds global online accessibility best-practice standards to ensure that the program is accessible to you. It is contrast and color-blind tested, allows for adjustments in font size, and is screen-reader friendly. For ease of use, we do suggest a caregiver be enrolled to assist you with the program.
