Launch Materials
When preparing for launch of a program, your Customer Success Associate (CSA) or Customer Success Manager (CSM) will provide various launch materials to help the team prepare for enrolling patients. See below for a list of materials and scripts available to help with your SeamlessMD launch and when explaining the program to patients.

Note: Please contact your CSA or CSM to request the materials that work best for you. The following materials should be tailored to take into consideration workflow decisions, type of enrollment, and the type of care plan that is available to patients.
1. Patient Brochure
This brochure is a 1 or 2-page insert that explains what SeamlessMD is, how to access an account, and how to use the program. We strongly recommend giving this brochure to all patients, caregivers, and family members.

2. Patient Sign-Up Sheet (Optional)
There is a sign-up sheet that can be given to patients or that your team can fill out when telling patients about SeamlessMD. It collects their consent and their information in case you want to enroll them at a later date. Some teams will use this to collect the patients information when they are in the office and then enroll patients at a set frequency (e.g. end of day, end of week).

3. Patient Script/Talking Points
This script offers talking points that can be used to inform patients about SeamlessMD, as well as how and why they should use the program. We suggest distributing it to anyone who will be interacting with patients concerning SeamlessMD, as having a unified message will drive patient participation and engagement.

4. SeamlessMD Posters
We offer digital patient posters that can be printed and displayed in clinics, waiting rooms, and other patient areas, to remind patients to sign up and use SeamlessMD throughout their peri-operative journey.

5. Provider Materials and Brochures
There is a brochure for hospital and clinic staff to provide them with a deeper understanding of SeamlessMD and to answer any questions that may arise if a staff member is not aware of the program.

6. Discharge Instructions
For surgical programs, we have developed an insert that can be included in patient discharge instructions to remind patients to use the program once they have gone home and to provide them with instructions for logging into their account.
