Manual Enrollment
Manual enrollment is a quick process that takes anywhere between 30-60 seconds per patient. To enroll a patient, you will first want to sign into your account using either https://ca.seamless.md or https://usa.seamless.md, depending on where your hospital is located.
Follow These Steps to Manually Enroll a Patient
- Select the 'Patients' tab at the top left of your screen to ensure you are on the patient roster dashboard
- Click on the green 'Add a Patient' button on the right

- This will open up the enrollment page. Fill out all of the patient's information, including:
- Patient Name
- MRN (or health card number)
- Contact information (email address or phone number)
- This is required in order for a patient to receive a notification they have been enrolled and to receive messages and reminders from SeamlessMD
- Caregiver information
- The care plan they are to be enrolled in and any care plan specific characteristics (if applicable - such as surgery sub-type, surgery approach, bowel prep)
- Physician
- Procedure date (if applicable)
Please note that if patient characteristics have been setup for the program during implementation, you will have to select the patient characteristic upon enrollment (eg: ERAS, procedure type, inpatient or outpatient)
- Click 'Save' to enroll the patient or click 'Save and Add Another Patient' if you are enrolling more than one patient at once


Other considerations when manually enrolling a patient:
- Refer to the article on Caregivers for information as to how to add the caregiver
- When entering the contact information, please ensure that the email address or phone number are entered correctly (you will see a notice that the email/phone number is either valid or invalid)
- You will want to toggle 'ON' if the patient wants to receive reminder
- You can enroll a patient without a surgery/procedure date. However, unless it is a chronic program, the patient will not progress through the program until a date is added
- You can add a discharge date if it is already known during the enrollment process (typically for same-day patients)
Please note that your CSA/CSM will provide you with training on how to enroll patients so it is best to follow the steps provided during training.
