Export Individual Patient Survey Data (PDF)

Note: Your CSM must first configure the specific survey (e.g. health check) to auto-generate a PDF before this feature can be used. It is only available for surveys that have been configured.

1. On the patient roster, click “View” for the patient profile you would like to export surveys from.

2. Once a patient completes a survey, it will automatically be generated into the tab “Patient Files” (if configured).

3. Click the file name to open the survey. This will open the individual survey and responses for the patient in PDF format to download/save.
