Available Data Exports
Note: All data on the exports will be based on the care plans and date filters you have selected.

| Patient List | List of patients and key information about their care plan (e.g. date of signup, date of setup survey completed, activation true/false, patient characteristics, provider name, etc.) |
| Setup and Final Surveys Data Export | Setup survey responses (tab starts with "SU") and main satisfaction survey (the satisfaction survey patients complete at the end of the program) responses (tab starts with "MS") The first tab will have the patient list. There will be a tab for each version of the survey (either for different care plans, or if changes were made to any of the surveys). |
| Post Discharge Health Check Survey Export | Daily health check responses and which days they were completed on for patients. The export will show one row per patient for each day the health check was available to them (e.g. a patient with a 30-day health check will have 30 rows in the export). The first tab will have the patient list. There will be a tab for each version of the health check per care plan selected. |
| Scheduled Surveys Export | Scheduled survey responses, including patient satisfaction, validated surveys, pre-operative surveys, etc. Note: it does not include repeating surveys or the daily health check) The first tab will have the patient list. There will be a tab for each version of each survey. |
| Repeating Scheduled Surveys | Survey responses for surveys scheduled on a recurring basis. Note: it does not include the daily health check. Use the "Post-Discharge Health Check Survey Export" instead. The first tab will have the patient list. There will be a tab for each version of each survey. |
| Opt Out Data | List of patients who have opted out of SeamlessMD, including the opt-out date and the reason they opted out. |
| Alerts Triggered Data | A list of alerts triggered by patients, including the status of the alert (open, in progress, resolved), the recommendation the patient received, and which user resolved the alert. |
| Follow-Up Module Data | A list of follow-up messages sent to and received from patients |
| Response Rate Data | Aggregated response rates for all surveys |
| Patient Measurements/Biometrics Data | Responses to any patient-submitted measurement data from a metric tracker (e.g. blood pressure tracker, heart rate tracker). Note: not all care plans include metric trackers. Biometric data collected in a health check are not included here. |
