Manually Completing a Patient's Care Plan

A patient's completion date (the end of their program) will be automatically set by the system based on the care plan criteria (e.g. if a surgical program is 30-days post-discharge, the patient will automatically complete their program 30-days after their discharge date).

There may be instances when you want to manually set a different completion date (e.g. after 14-days,  they no longer want to continue with the program). Manually setting their completion date will move them to the end of their program and preserve their data in any reports/analytics. This can only be done early; you cannot set a completion date to extend the length of their program.


  1. From patient roster, find the patient and click “Edit”. 

  1. Scroll down to the section called “Surgery/Procedure Details” and fill in the end date under “Completed On”. 

  1. Click “Save”. The patient’s care plan will now be completed (i.e., archived), and they will not receive any email/text reminders, unless the program includes messages outside of the formal program length (e.g. a validated survey sent 1-year after surgery). The completed date is noted in their profile details.

  1. To filter for completed patients, go to the patient roster and click “Archived” on the quick filters.
