About the Patient Roster
When you first log into your account, you will see the patient roster. The patient roster contains a full list of the patients enrolled on to the program. It includes a a view of each patient with columns that align with patient information. Please refer to the screenshot below for a breakdown of the components available within the patient roster.
Patient Roster

Patient ID: a de-identified patient number associated with each patient that can be used to communicate outside of the organization or with SeamlessMD as it is not considered PHI
MRN: the patient's Medical Record Number
Info: this column will include a notice if the patient has not opened their setup email or if the setup email has not been delivered
Care Plan: refers to the program that the patient is enrolled on to (i.e. hip, colorectal, maternity, etc.)
Provider: refers to the surgeon/physician listed on the patient's account
Days Since Sign Up: the number of days since the patient has been enrolled on to the program
Enrollment Date*: the date that the patient was initially signed up for the program
Channels: please note that you may see a column called 'Channels' on the patient roster. This column indicates what form of contact information has been linked to the patient's account.
- Phone Icon: patient has a valid phone number in the system
- Envelope Icon: patient has a valid email address in the system
- Person Icon: patient has a caregiver in the system
- Computer Icon: patient has activated their account using a web browser
Procedure Date: the patient's surgery/procedure date (or due date if on the maternity care plan), which appears upon enrollment
Discharge Date: hospital discharge date (entered by patients for non-integrated site, from ADT feed for integrated sites)
Post-Discharge Day: days since discharge from the hospital
Program Length*: the total length of the SeamlessMD program (after which point the patient is considered completed)
Last Survey: the date of their last survey completion
Last Survey Status*: displays survey recommendation (e.g. all clear) for the last survey submitted (inclusive of any and all surveys in the program (e.g. set up survey, satisfaction)
Last Health Check Status*: displays survey recommendation (e.g. all clear) for the last Health Check submitted (limited to the Health Check survey only)
Unresolved Issues*: displays a count of the number of open or in-progress alerts that have not yet been marked as resolved (refer to the 'Monitoring' articles for more information)
* These columns are optional to add to your roster view. Contact your CSA/CSM if you would like to add them.
