Internal Launch Announcement Email Templates
As you prepare to go-live with SeamlessMD, it's important to generate internal excitement and awareness of the program. Below are a few email templates you can use to share information about SeamlessMD with the department.
Update any text in square brackets ([ ]) with the relevant information.
Kick-Off Announcement Email to Department
Date: Week Before Kick Off
Hello everyone,
[Organization name] has recently partnered with SeamlessMD to enhance patient care for [CARE PLAN] patients.
SeamlessMD is a digital care journey platform that guides patients through their healthcare journey whether that's pre-/post a post-procedure, or managing a chronic condition. We ultimately chose to partner with SeamlessMD to [include key goals/objectives/etc.].
The platform is available to patients on their own devices (e.g. phone, tablet, computer) and provides:
- customized reminders and to-do items
- educational content covering clinical and non-clinical topics
- health tracking to report on symptoms they may be experiencing so they can receive tailored feedback to help them manage at home
We are kicking off the implementation of this project on [KICKOFF DATE]. We have identified a core group of clinical team members to support the implementation process, but we want to make sure that everyone is aware of the program and has an opportunity to provide feedback if they wish to do so.

First of all - a big thanks to the implementation team, comprised of [NAMES] who worked diligently to create a successful care plan for our patients. A thank you as well to all periphery members who gave feedback throughout the process, and to all team members who are supporting the rollout.

I’ve attached a brochure with more information about SeamlessMD and how it’s being used with patients at [Organization name]. When speaking to patients who are participating in the program, please encourage/remind them to leverage the information available on the platform. If you would like some talking points, please refer to this document.
Patients will be enrolled in the program at [DETAILS ABOUT ENROLLMENT]. [A monitoring team will also be receiving alerts if a patient reports more concerning signs/symptoms so we can intervene and connect with the patient to provide the necessary care.]
