What Patient Data We Collect on SeamlessMD

To register patients, we typically collect the patient's surgery/procedure date (if applicable), their first and last name, their MRN, and their provider's name. Other data may also be collected depending on the program (e.g. date of birth, discharge date).

Each care plan collects a variety of information via surveys sent to patients. It may include:

• A set-up survey to collect patient demographics, comorbidities and risk factors
• Health checks to collect general recovery data (i.e. pain, mobility, signs of infection) and provide personalized recommendations on how to manage concerns
• Validated Surveys (e.g. PROMIS-10) to collect patient-reported outcomes such as pain, impact on daily life, mood/anxiety, recovery metrics, etc.
