Best Practices for Increasing Enrollment and Engagement
- Enrollment on to SeamlessMD is framed as part of the normal standard of care and as a regular process during the patient's visit
- Patients can "opt out" of the program and are not asked if they want to "opt in"
- Individuals educating and enrolling patients offer examples of what other patients like about the program (i.e. they like that it helps them stay connected to their care team, patients like being able to send wound photos)
- Provide all patients with the patient brochure to take home
- Put up posters within the waiting room and around the hospital about SeamlessMD
- Make sure that everyone in the clinic has at least a basic understanding of what SeamlessMD is so that if patients ask, they can answer or find someone else who knows the answer
- Care team's phone number is provided within the program for patients to be able to call
