4. Patient Enrolled on to Incorrect Care Plan

Once a patient has been enrolled onto the program, the care plan cannot be changed. However, we can delete this patient so they can be re-enrolled onto the correct care plan.


Please contact support@seamless.md if you have enrolled a patient on to the incorrect care plan and please confirm that we can delete their account from the program. Once the account has been deleted, the patient's information will be removed from the program and they can be re-enrolled on the appropriate care plan.
