Export Ontario Health Reports
- From the Patient Roster, click “Export” (top right corner).

- Scroll down to the ‘OH Export’ section and select the appropriate month you would like to report.
- Click “Download Data (.xlsx)”. This export will contain all active care plans. If certain care plans need to be excluded from OH reporting, delete those rows before submitting.

- Your export will begin to download, and will be available at the top of your page. It may take up to 30 minutes to receive your file. Refresh the webpage and click “Download” when the report becomes available. Note: This export contains Protected Health Information (PHI).

- Upon opening the report, you will see the following 3 tabs:
- Activity (contains any patient sent a daily health check in that month)
- Discharge Summary (contains any patient who completed the program in that month)
- Patient Experience (contains any responses to the OH satisfaction questions submitted in that month)
- Fill out the missing information (Org ID, Patient Escalations & Encounters) across the tabs that are not collected within SeamlessMD. Note: solution escalations and any escalations documented using the alert statuses are captured already.
- Review & delete unnecessary columns (highlighted in yellow) before dissemination. Additional columns have been included to help you match patients in your EMR to fill out the encounters and escalations. Columns to be deleted include: patient last name, MRN, email, booking ID, and Setup Survey completed.
