How to Enroll a Patient on to a 2nd Program (Multiple Episodes of Care)
Have you received this error message when attempting to create a patient account?

This means the patient has already used SeamlessMD for a previous procedure and was enrolled using the same email address or phone number you are attempting to use. The system does not allow the same email address or phone number to be associated with more than 1 account.

Follow These Steps to Enroll a Patient on to a 2nd Account
- From patient roster, find the patient and click “View”.
- Scroll down to the section called “All Linked Patient Accounts” and click “Add New Episode Of Care”.
- Select the procedure. The patient’s name and contact information will be pre-populated from the previous account. Set the surgery/procedure date and select the surgeon. You may have to select other patient characteristics depending on the care plan (e.g. location, sign-up type, etc.).
- Click “Save”. A new account will be created for the patient’s new procedure. The patient will receive an email with the link to activate their account. If the patient has an overlapping SeamlessMD program from another recent procedure, they will be able to toggle between their two care plans.
