Viewing SeamlessMD Reporting Dashboards

SeamlessMD provides a reporting dashboard to help you understand patient engagement and clinical trends.

To access the aggregate reports, log in and click "Dashboard" from the top menu.

Home Tab

Under the “Home” tab you will see a summary of patients who have been enrolled in SeamlessMD by phase. Select ‘procedure set filters’ and then ‘apply’ to filter by care plan. You will see the date and time the data was last updated.


Reports Tab

The “Reports” tab includes a number of sub-tabs to view patient engagement and satisfaction data.

  1. Select the care plan(s) and time frame (a specific date range or report mode to view monthly/quarterly comparison).
  2. Use the Advanced Filters to filter by specific providers and/or certain patient characteristics.
  3. Click "Apply".

Note: Except for the  "Enrollment" tab, this data is based on patients who completed the program in the specified time period. On the "Engagement" tab, toggle to "current patients" to view results based on the activity in the specified time period (e.g. surveys sent).


Summary of Each Tab (See more details below)

Sub Tab Summary of Data
Summary: a summary of key enrollment and engagement statistics shown monthly or quarterly. This includes:  enrollment trends, patient activity, and satisfaction survey data. 
Enrollment: Key enrollment statistics, including enrollment trends and enrollment broken down by provider, care plan phase, user and patient characteristics (if applicable). 
Engagement: A breakdown of patient engagement on the program. This includes the percentage of patients using the program pre and post surgery, survey response rates, and clinical alerts activity. 
Satisfaction: Key SeamlessMD satisfaction data (including patient-reported: would recommend Seamless, felt more confident before/after surgery etc) as well as all free-text comments. 
Clinical Trends: Trends for patient-reported outcomes tracked throughout the post-discharge phase, generally including:  anxiety, pain level, minutes walked, opioid intake etc (one graph per PRO, and PROs displayed may vary per program). 
Outcomes: SeamlessMD outcomes data (including patient-reported: phone calls avoided, ER visits avoided, length of stay and health care service usage) as well as all free-text comments. 
Compliance: Summary of patient-reported compliance with clinical protocols (e.g. ERAS instructions), broken down by each phase (preop, in-hospital, and post-discharge patient). Only applicable if your program is tracking compliance.
Health Check Recommendations: Distribution of post-discharge Health Check recommendations given to patients at the conclusion of each health check (e.g. 12% of submissions ended in ‘call your surgeon’s office), with details on the most common triggers for each recommendation and the total frequency of each.

Summary Tab: Use the Summary tab for a quick view of key enrollment, activity and satisfaction data to see program performance.

Enrollment Tab: View a breakdown of enrollment trends including enrollment by: 

  • Provider: A patient’s surgeon or care provider
  • Care plan Phase: When a patient was enrolled onto the program (i.e. pre-op, in-hospital, post-discharge)
  • User: Individual(s) that enrolled patients
  • Patient characteristics: Surgery type, Hospital Site, etc. (if applicable)

Engagement Tab: Understand how patients are engaging with the program throughout their care journey. View data on:  

  • Holistic Engagement - Patients who logged any type of activity each week in the program, including logging in, viewing education, checking off a to-do list item, or completing. 
  • Health Check Engagement - Patients who completed the post-discharge health check. 
  • Survey Response Rates - Patients who responded to surveys in the program broken down by one-time surveys (i.e. satisfaction surveys) and repeating surveys (i.e. validated surveys). 
  • Clinical Alerts Activity - Number of alerts triggered, applicable for monitored programs only 
  • Wound Photo Submissions - Number of wound photos submitted by patients

Satisfaction Tab: View a breakdown of the data collected through the patient satisfaction surveys, including the rate of reduced anxiety or improved confidence through the care journey, recommendation rate, and ease of use. 

Read patient comments, including thank you messages and feedback on the program.

Clinical Trends Tab: Gain actionable insights from the Clinical Trends tab to understand population trends from patient-reported outcomes, such as how wound pain improves after surgery. 

Any PRO question included in the program can be included here, including mood, anxiety, walking trends, wound pain, etc.

If multiple care plans are selected, PRO trends for each can be compared in one graph.

Outcomes Tab: From the outcomes tab view patient reported data on: 

  • Call and visit avoidance - Patients who reported the program helped them prevent 1+ phone call or visit. Free text responses on what patients would have had to call or visit for are included. 
  • Length of stay - Calculated based on the surgery and discharge date entered in SeamlessMD
  • Health care service utilization - Patients who selected a health care service was used within the daily health check, broken down by number of instances and number of unique patients

Compliance Tab: preop, in-hospital, and post-discharge compliance rates with protocols (eg.g ERAS). Only applicable if you are tracking compliance

Health Check Recommendations Tab: View the distribution of health check recommendations provided to patients, along with details on the most common triggers for each recommendation and the total frequency of each. 

Use this data to understand the most common problems for patients,  and areas where additional education may be useful.

Surveys Tab

Select the “Surveys” tab to view aggregate responses to individual survey questions in any of the surveys (e.g. Post-Discharge Health Check, 14-Day Satisfaction survey). Select the care plan and the survey you would like to see responses for and the data will automatically load.

  • Note: this data is available in real-time and includes all survey responses
